Chronic Health Care Plans, also known as GP Management Plans are available for our regular patients with ongoing medical conditions lasting more than 6 months.

A Care Plan is a written plan between patient and GP to help best manage your medical condition. Including co-ordinating care with relevant allied health professionals.

All Chronic Health Care Plans are bulk billed by your GP at Hornsby Fountain Medical Centre.

Care Plan Reviews
Once a Care Plan is in place, it should be regularly reviewed by your GP. This is an
important part of the plan cycle, to help achieve the goals and review results and make changes if necessary.

A Care Plan review is usually undertaken every 3 to 6 months.
